500 Victory Road, Quincy

From Points NORTH:
Take SOUTHEAST EXPRESSWAY/I-93 S to QUINCY/NEPONSET EXIT 12. Stay RIGHT as you come off ramp. Pass in front of Staples/Walgreens, go under Expressway and bear RIGHT onto NEPONSET RIVER BRIDGE. (Do not take Rte. 3A south). Take RIGHT off bridge marked MARINA BAY and follow, for approximately 1 mile. Turn RIGHT onto Seaport Drive. Enter next roundabout and take 2nd exit onto VICTORY RD.

From Points SOUTH:
Take MA-3 NORTH to I-93 N (Southeast Expressway) to EXIT 11 GRANITE AVE. At end of ramp, follow Granite Ave, over drawbridge to first set of lights. Turn right onto Hilltop Street. Turn RIGHT at first set of lights onto GALLIVAN BLVD. Pass in front of Staples/Walgreens. Go under Expressway and bear RIGHT onto NEPONSET RIVER BRIDGE. (Do not take Rte. 3A south). Take right off bridge marked MARINA BAY and follow for approximately 1 mile.Turn RIGHT onto Seaport Drive. Enter next roundabout and take 2nd exit onto VICTORY RD.

From MASS PIKE EAST:
Take I-90 East all the way to exit 24 A-B-C on the LEFT toward I-93 NORTH/CONCORD NH/S. STATION/I-93 SOUTH/QUINCY.
Take the 24C-B exit on the LEFT. Merge onto I-93 S/US-1 S/MA-3 S via EXIT 24C on the LEFT toward QUINCY. Turn slight right onto GALLIVAN BLVD/MA-203 E. Turn slight right onto NEPONSET AVE/MA-3A S. NEPONSET AVE/MA-3A S becomes NEPONSET BRIDGE. Turn slight LEFT onto QUINCY SHORE DRIVE. Take the ramp toward MARINA BAY. Keep RIGHT at the fork to go on COMMANDER SHEA BLVD. Turn RIGHT onto Seaport Drive. Enter next roundabout and take 2nd exit onto VICTORY RD.

PUBLIC TRANSPORTATION:
Take Red Line to North Quincy. Take Bus #211. Bus leaves at 10 minutes before the hour. No Sunday service. Take bus to Marina Bay stop. Marina is 15 minute walk from bus stop. Last bus back to North Quincy T Station leaves at 6:15 PM. A & A Metro South, a courtesy shuttle provided by the business community at Marina Bay, departs the North Quincy T Station (parking lot-lower level) every 20 minutes from 6:30AM to 8:30PM. The Marina Bay location for pick-up/drop-off is the front entrance to 500 Victory Road. There is no fee for this shuttle service.

859 Willard Street, Quincy

Directions:
This site is accessible from all major highways including Routes 95 and 93.

From Points North:
Take Southeast Expressway (Rte. 93 South) to Exit 6. At the lights at the end of exit ramp, turn left onto Granite Street (Route 37). Go straight through the set of traffic lights by Herb Chambers Ford. Granite Street becomes Willard Street at this point. You will see two large office buildings on your left, directly across from Ethan Allen furniture. Take left into parking lot. One Adams place is the building right in front of you. Bayview Associates is on the fourth floor, to your right after you exit the elevators.

From Points South:
Take Route 3 North to Route 93 South. Take Exit 6. At the lights at the end of exit ramp, turn left onto Granite Street (Route 37). Go straight through the set of traffic lights by Herb Chambers Ford. Granite Street becomes Willard Street at this point. You will see two large office buildings on your left, directly across from Ethan Allen furniture. Take left into parking lot. One Adams place is the building right in front of you. Bayview Associates is on the fourth floor, to your right after you exit the elevators.

460 Quincy Avenue, Quincy

Directions:
This site is accessible from all major highways including Routes 95 and 93.

From the center of Quincy:
Take Hancock Street. Hancock Street becomes Quincy Avenue. Follow Quincy Avenue through three sets of lights. After you pass through your third set of lights (there is a Walgreens drugstore on your left), you will see the entrance to 460 Quincy Avenue marked by a brown sign that reads "Quincy Mental Health Center/South Shore Mental Health." If you get to another set of lights, you have gone too far. Turn left at the entrance marker and go all the way to the top of the hill and park in the upper parking lot. Enter the building through the side entrance onto the third floor (directly across from the parking lot). On you immediate left is the Central Registration office. All SSMH services are available on the third floor. An information desk and elevator service are available from this entrance. Parking for individuals with disabilities is available at the front of the building.

Public Transportation:
The MBTA red line train offers service to Quincy Center, which is approximately 3 miles away from 460 Quincy Avenue. The MBTA offers bus service from the red line "T" stop and throughout the city of Quincy. Bus #225 regularly stops at the corner of Quincy Avenue and Howard Street by the Fore River Shipyard. This stop is located across the street from the entrance to 460 Quincy Avenue. The Ride bus line offers discounts and services to disabled seniors. For more information, call (617) 222-5123. 8-12 Hancock Court, Quincy

From Route 93 North:
Take the Quincy Center exit. Follow directions below.

Routes 3 North, 3 South, and 93 North all connect with Burgin Parkway. At the fourth set of lights, take a left. You will be driving on a very short road between two shopping plazas. At the lights at the end of this road, take a left onto Granite Street. Take your next left onto Fort Street. Take a left at the bottom of the hill. Go straight through the next set of lights. At the next set of lights, which will be blinking, take a left onto Hancock Court. Take the first right onto a narrow road. 12 Hancock Court is ahead on your right.

12 and 16 Moon Island Road, Quincy

Directions:

From 93 North:
Take exit 8, Furnace Brook Parkway. Take Furnace Brook Parkway and go through 6 lights. At the 7th light, take a left onto Quincy Shore Drive. Stay on Quincy Shore Drive until you get to the East Squantum Street intersection with Dunkin Donuts and a Getty gas station. Turn right at this intersection onto East Squantum Street. Pass Marina Bay on your left. Road will fork to right - stay straight onto Moon Island Road. Turn left into the parking lot. You will see three buildings - VFW straight ahead, 16 on your left, and 12 to the far left.

From 93 South:
Take exit 12, Neponsit/3A. On the exit ramp, stay to your right and follow signs to 3A/ Hancock Street. Get on the Neponset River Bridge and stay to the left. This will become Quincy Shore Drive. Stay on Quincy Shore Drive until you get to the East Squantum Street intersection with Dunkin Donuts and a Getty gas station. Turn right at this intersection onto East Squantum Street. Pass Marina Bay on your left. Road will fork to right - stay straight onto Moon Island Road. Turn left into the parking lot. Routes 3 North, 3 South, and 93 North all connect with Burgin Parkway. At the fifth set of lights, take a right onto Granite Street. Take an immediate right into the parking garage. Parking for less than two hours is on the ground floor. Parking for more than two hours is on the second floor and up. Walk to where you pulled into the garage and go right when you get to the sidewalk. Take a right onto Hancock Street when you get to corner. 1431 Hancock Street is about four doors down on the right. The entrance is set back from the street and is between J's Variety Store and a nail salon. Take the elevator or the stairs to the third floor.

Plymouth Office

64 Industrial Park Road, Plymouth

Directions:

From Route 3 South:
Take exit 8. At the end of the ramp, take a left. Proceed approximately 1.3 miles and take a right onto Industrial Park Road. Office is located on your left hand side. Parking is available at the front and back of the building.

From Route 3 North:
Take exit 8. At the end of the ramp, take a right. Proceed approximately 1.3 miles and take a right onto Industrial Park Road. Office is located on your left hand side. Parking is available at the front and back of the building.

Public Transportation:
The MBTA has extended its red line train service into Plymouth. Plymouth Are Link (PAL) offers bus service from the MBTA stop at Cordage Park to the Peter Pan & Brockton bus terminal in the Industrial Park, which is only three buildings down from SSMH (just under one quarter of a mile away). PAL also runs their transportation services throughout Plymouth weekdays from 6:25 am to 6:20 pm. Freedom Link stops include: Algonquin Heights Apartments; the Plymouth Pulic Library;Jordan Hospital; the old Post Office on Main Street; Cordage Park; and Independence Mall. In addition to all these stops, the Liberty Link also picks up at the CVS on Main Street and the Armstrong Ice Rink. Both links stop at the P&B bus terminal in the Industrial Park. Cape Offices

310 Barnstable Road, Hyannis

Directions:

From Greater Boston/Southeast areas:
Take Route 3 South to the Sagamore Bridge. Take Route 6 East to exit 6. The ramp will spill out onto Route 132. Bear right, following Route 132 for about 5 miles towards Hyannis Center. You will pas the Cape Cod Mall before reaching a rotary. Once on the rotary, take your second right onto Barnstable Road. We are located less than a quarter mile on your left after a KFC Restaurant.

From Route 6 West:
Take exit 6 and bear left off the ramp towards Hyannis. You will past he Cape Cod Mall before reaching a rotary. Once on the rotary, take your second right onto Barnstable Road. We are located less than a quarter mile on your left after a KFC Restaurant.

Public Transportation:
Two bus lines - Plymouth & Brockton and Bonanza - run regularly up and down the Cape, stopping and picking up daily at the Plymouth & Brockton bus terminal in Hyannis. The bus terminal, which is a half mile from our office, is located at 17 Elm Avenue. When leaving the terminal, take a left onto Elm Avenue. Take a right onto Barnstable Road. Follow the road to our office, which is located on the right hand side. If you reach the rotary you have gone too far. For information on bus schedules, call (508) 775-6502. The Cape Cod Regional Transit Authority Operation offers door-to-door service via their B bus service. Discounts are available for seniors and individuals with disabilities. 166 Depot Road, Chatham

Directions:

Take Route 28 to the Chatham rotary. Go 3/4 of the way around the rotary and exit the rotary at Old Harbor Road. Take your first left onto Depot Road. The Monomoy Community Services building is the first building on your left.

Wareham Office

215 Sandwich Road, Wareham

Directions:

From the Cape:
Go over the Bourne Bridge. Take the second exit— Wareham. At the lights, take a right and stay to the left. Follow Route 6 to the left, go through the next light. Bayview Associates is 1/4 mile up on the left hand side, across from Mr. K's art store.

From New Bedford and points West:
Take Route 195 East towards the Cape onto Route 25. Take the first exit and turn right at the second light onto Route 6 West. Bayview Associates is 1/4 mile up on the left hand side, across from Mr. K's art store.

From points North:
Take Route 495 South — this will become Route 25. Take exit 1 from Route 25 — Wareham, Buzzards Bay, Onset. (Please note that there are two exits 1 on this strip of road — the first exit 1 is for 495 South. The second exit 1 is for Route 25. You want to take the second exit 1.) Follow the exit ramp to the right and takea right at the second light. Bayview Associates is 1/4 mile up on the left hand side, across from Mr. K's art store.
